In the meantime

When you book your first appointment, just let us know you're a new patient — we'll have your paperwork ready when you arrive, and it only takes a few minutes to complete in our comfortable reception area. Or call us at (408) 996-0176 and we can walk you through anything in advance.

What to bring

  • A photo ID
  • Your dental insurance card, if you have one
  • A list of current medications
  • Any recent dental X-rays, if available (or we can request them for you)
  • A family member or trusted person who helps you make health or financial decisions, if you'd like them involved in planning your care
  • Patients under 18: a parent or legal guardian, who will need to sign consent forms

Frequently asked questions

Do I have to fill out forms before my appointment?

No, but it saves time. If you can't complete them ahead, arrive 15 minutes early and we'll help you finish them in our reception area.

How long does a first dental visit take?

Usually 60–90 minutes, including your exam, X-rays, and time to talk through what we find.

Do you accept new patients?

Yes. We welcome new patients age 5 and older, and consultations are complimentary.

Can I bring X-rays from my previous dentist?

Yes. Recent X-rays can save you from retaking them. Bring copies, or give us your previous dentist's contact information and we'll request them.

Is my information kept private?

Yes. Your forms and health information are protected under federal privacy law (HIPAA) and our Notice of Privacy Practices.

What if I need to reschedule?

Please call (408) 996-0176 at least 48 hours ahead so we can offer the time to another patient.
